Appointment- what you need to know

What To Expect:
My office is accepting new patients-both for patients who have insurance and those who do not. For new patients I request you fill out an information sheet(you can download the sheet in PDF- look under New Patient Information heading). If you have insurance please bring your insurance card so we can make a copy to file the insurance correctly. If you do not have insurance we request payment for the exam that day. The charge for the exam will depend on the extent of of the exam. Please call my office to get an estimate of your cost.If you currently wear contacts lenses please bring the prescription lens bottle/box with you. There is a $15 charge to examine contacts for correct prescription/fitting.

Preprocedure:
If you need a referral from your insurance company please bring this with you.(If you don't know if you need a referral call the number on your insurance card).When you arrive at the office the information sheet should take about 5 minutes to fill out( unless you download sheet PDF- look under New Patient Infomation) You can expect to be seen at your appointment time unless I have an emergency patient ahead of you.

Postprocedure:
If your eyes are dilated with drops they will be sensitive to sunlight and your reading vision will be blurred for several hours. We will give you temporary sunglasses after your exam. Not every patient needs to be dilated each time you have an exam.

Staff Information:

Miriam Reese has worked in my office since 1985. Her hours are 8:30AM- 4:30 PM Thursday, & Friday.She schedules appointments, answers all the billing and insurance questions, refill prescriptions for glasses, contact lenses and medicines, give directions to the office and creates a pleasant atmosphere in our office. Andrea Phillips works Monday, Tuesday, Wednesday from 8:30- 5:00PM. She makes appointments answers insurance and billing questions, can fax glasses or medication refills. Both Miriam and Andrea are valued staff in my office.
